Student Affairs and Conduct Reporting

Keyano College is committed to maintaining a respectful, inclusive, and safe learning environment. To ensure that concerns are addressed appropriately, the following online reporting forms are available: 

Students, Volunteers, and Visitors can tell us about your concerns here: Campus Experience Report (for Students & Visitors) 

This form is intended for students, volunteers, and campus visitors who wish to report on: 

  • Academic Integrity: (Examples may include: Cheating, plagiarism, falsifying data or records, unauthorized collaboration, exam fraud, or other breaches of academic integrity.)  

  • General Complaint: (Examples may include: Dissatisfaction or concerns about services, academic issues, course management, facilities, treatment by staff/faculty, or administrative practices, academic accommodations issue.)  

  • Non-Academic Misconduct: (Examples may include: disruptive behavior, vandalism, theft, drug/alcohol violations, breaches of college policy unrelated to academic work, behavioral misconduct, bullying, harassment, discrimination, gender-based or sexual violence, sexual harassment, assault, stalking, coercion, dating violence, domestic violence, exploitation, non- consensual sexual acts, or related misconduct.)  

  • Appeal Academic Standing Decision: (Requests for appeals related to being placed on academic probation or suspension because of GPA.) 

  • Appeal of Previous Decision: (Requests for appeals are related to Academic Misconduct sanctions, Non-Academic Misconduct sanctions, outcomes of formal complaints, denial or implementation of academic accommodations, or disciplinary outcomes. Appeals typically cite concerns like procedural unfairness, bias, new evidence that was unavailable previously, or sanctions considered disproportionate.) 

  • Wellness Check: (Requests for a Mental Health Coordinator to call or connect with someone you have concern about). 

  • Anonymous Gender-Based and/or Sexual Violence Reporting and Support: Students experiencing Gender-Based and/or Sexual Violence have the option to disclose, report or communicate anonymously with specially trained support staff here: keyano.reessecure.com

This form is for Keyano College employees who wish to report concerns regarding the conduct of students, volunteers, or visitors that may be contrary to student- or community-related policies. 

  • Submit a completed Informal Resolution Form for Academic Misconduct to be logged. 

  • Report Academic Misconduct(Examples may include: Cheating, plagiarism, falsifying data or records, unauthorized collaboration, exam fraud, or other breaches of academic integrity.) 

  • Report Non-Academic Misconduct (Examples may include: disruptive behavior, vandalism, theft, drug/alcohol violations, breaches of college policy unrelated to academic work, behavioral misconduct, bullying, harassment, discrimination, gender-based or sexual violence, sexual harassment, assault, stalking, coercion, dating violence, domestic violence, exploitation, non-consensual sexual acts, or related misconduct.) 

  • Request a Wellness Check: (Previously known as an Early Alert, let a Mental Health Coordinator know if you have concerns about a student's mental well-being for follow up within 24 business hours.) 

  • Refer to Academic Success Coach: This for students who are needing assistance with academics 

Please note: This form is not for reporting concerns about employees. Employee-related concerns must be directed to People & Culture.

Personal information collected through these forms is gathered under the authority of the Post-Secondary Learning Act (section 65) and the Freedom of Information and Protection of Privacy (FOIP) Act (section 33c).
