This form is intended for students, volunteers, and campus visitors who wish to report on:

Academic Integrity: (Examples may include: Cheating, plagiarism, falsifying data or records, unauthorized collaboration, exam fraud, or other breaches of academic integrity.)

General Complaint: (Examples may include: Dissatisfaction or concerns about services, academic issues, course management, facilities, treatment by staff/faculty, or administrative practices, academic accommodations issue.)

Non-Academic Misconduct: (Examples may include: disruptive behavior, vandalism, theft, drug/alcohol violations, breaches of college policy unrelated to academic work, behavioral misconduct, bullying, harassment, discrimination, gender-based or sexual violence, sexual harassment, assault, stalking, coercion, dating violence, domestic violence, exploitation, non- consensual sexual acts, or related misconduct.)

Appeal Academic Standing Decision: (Requests for appeals related to being placed on academic probation or suspension because of GPA.)

Appeal of Previous Decision: (Requests for appeals are related to Academic Misconduct sanctions, Non-Academic Misconduct sanctions, outcomes of formal complaints, denial or implementation of academic accommodations, or disciplinary outcomes. Appeals typically cite concerns like procedural unfairness, bias, new evidence that was unavailable previously, or sanctions considered disproportionate.)

Wellness Check: (Requests for a Mental Health Coordinator to call or connect with someone you have concern about).

Anonymous Gender-Based and/or Sexual Violence Reporting and Support: Students experiencing Gender-Based and/or Sexual Violence have the option to disclose, report or communicate anonymously with specially trained support staff here: keyano.reessecure.com
