Work Request Form

  • Submit a Work Request to FacilitiesHelpdesk@keyano.ca if you need something cleaned, repaired, installed, moved, replaced, or for key requests.
  • Work Requests are processed for all maintenance, grounds, and custodial needs.
  • Emergency Work Orders are quickly identified, routed to the appropriate staff, and will be completed within 24-48 hrs.
  • Between 8:30 to 4:30 p.m. Monday - Friday, complete a Facilities Work Request.  After hours, an emergency should be reported to Security at 4911.

Renovations and Site Improvement Requests

  • To request an improvement or change an existing area, complete a Facility Renovation Request Form (Gen.08) as per policy 6.46 by November 1st.
  • The form must be submitted and approved by the department chair, department head, or program director and submitted to the Vice President or President for review during the Budget review process in December.<
  • The Director of Facilities will receive your request in early January and will proceed to review the viability of the request and implement a cost study.
  • You will be notified about the decision prior to the end of Winter Term.  At that time, a project manager will be identified to contact if you have questions.
  • A Facilities Renovation Request submitted after the deadline will be considered individually, based on urgency, cost, and budget.  Furniture requests must be approved through the budget process.

Please clearly identify the Custodial requirements for all requests.
